BECOMING A PATIENT
Our goal at Integrative Cardiology is to provide you with the highest level of personalized care. We are committed to supporting your efforts at achieving optimal health and preventing disease.
Initial Comprehensive Consultation
Prior to your initial consultation you will be asked to complete a comprehensive online Health Questionnaire, as well as submit health records and relevant diagnostic tests. The Health Questionnaire and records will be reviewed beforehand by the physician and health coach. During the consultation, you will have the opportunity to discuss your concerns and goals, and provide detail to fill in any gaps not covered by the Health Questionnaire. You will also undergo a physical examination. Using the information gathered, you will receive a detailed, customized plan to fit your individual preferences and goals.
Investment: $1,695 ( $500 non-refundable deposit)
Initial Basic Consultation
You may choose to forego the comprehensive consultation and meet with the physician for a time-based consultation to discuss your concerns and goals and together develop a plan.
Investment: $500 per hour prorated ( $250 non-refundable deposit)

Integrative Cardiology is an insurance-free practice. We believe the best way to care for our patients is to use the most effective interventions from both conventional and holistic medicine.

Our office accepts cash, check or credit cards. Payment for the initial consultation is expected at the time of your appointment.
Obtain your medical records, lab test results, and pertinent diagnostic test reports from your physicians. Please send to Integrative Cardiology, at the earliest possible time, prior to your appointment. Please contact your physician or other health care provider to obtain these records.

Confirmation and cancellation of appointments
Due to the extensive preparation involved in preparing for a consultation, there is a 72-hour cancellation policy for all appointments. You must cancel at least 72-hours prior to your appointment or you will forfeit the non-refundable deposit. You may cancel your appointment by calling the office or by emailing firstname.lastname@example.org
Integrative Cardiology is an insurance-free practice. If you have out-of-network-coverage we will provide you with a detailed billing summary which you can submit to your insurance company for reimbursement. Please note that there may be procedures and laboratory tests that are non-covered due to your individual policy/plan type. If you have any questions regarding your medical coverage, please call the telephone number on the back of your insurance card.
Integrative Cardiology does not participate in the Medicare program. If you are a Medicare Part B beneficiary and wish to become a patient of Integrative Cardiology, you are required to accept the terms and conditions set forth in a Private Contract between you and Integrative Cardiology. This Private Contract provides that absolutely no Medicare payment will be made to you or to Integrative Cardiology for the services provided, even if such services are covered by Medicare. Under the Private Contract, you acknowledge that you accept full responsibility for the payment of charges for all services rendered by Integrative Cardiology; such payments are due in full at the time of service.

Prescription refill requests
It may take up to 5 business days to process a prescription refill. Please plan ahead to avoid any interruptions in your medications. Prescription refills can be faxed to our office by your pharmacy. Our fax number is (551) 313-0003.
