The Process
We believe the best way to care for our patients is to use the most effective interventions from both conventional and holistic medicine.
Prior to your initial consultation you will be asked to complete a Health Questionnaire, as well as submit health records and relevant diagnostic tests.
During the consultation, you will have the opportunity to discuss your concerns and goals, and provide detail to fill in any gaps not covered by the Health Questionnaire.
Using the information gathered, you will receive a detailed, customized plan to fit your individual preferences and goals.
Things to Know
Medical Records + Payment
Medical Records
Obtain your medical records, lab test results, and pertinent diagnostic test reports from your physicians. Please fax the records, at the earliest possible time, prior to your appointment. Contact your physician or other health care provider to obtain these records.

Fees
Scheduling Fee
There is a $100 scheduling fee for initial consultations that is nonrefundable. This
will be deducted from the cost of the Initial Consultation fee.
Initial Consultation: $750
This includes: 60 minutes of face to face time, review of pertinent medical records, health and symptom questionnaire forms, formulation of a detailed management plan, ordering diagnostic assessments based on shared decision making.
Face to face time that exceeds the 60 minute limit is charged at $500/hour prorated.
Physician Services: $500/hour prorated
Provider services may include time spent on face to face telemedicine consultations, reviewing and responding to email questions, reviewing records, preparing requisitions, filling prescriptions, performing research upon request, communications with other providers, etc.
Keeping In Touch
Communication
To protect your privacy & confidentiality of protected health information, all communication will need to go through our HIPAA-compliant patient portal system called Healthie. This is where you will:
Communicate via the messaging option
Fill out your intake forms
View summaries of our sessions as well as any additional educational resources
Upload any pertinent lab results, documents, photos or written journal entries
Journal your food intake during the week
Access the link for our virtual Zoom sessions within the calendar option
On Communication
Important Note
Due to our respect for your time and need for answers to your questions, our policy is to respond to any communication within a 24 hour period. Exceptions include holidays, weekends, and vacation time.
If you are experiencing a medical emergency, please call 911 or go directly to the nearest emergency room.
For other immediate concerns you may call 646.866.1900.
If we are unable to answer your call, please leave a message with your name, date of birth, phone number, and reason for calling and we will get back to you as soon as possible.
Check Your Coverage
Insurance Information
Integrative Cardiology is an insurance-free practice. If you have out-of-network-coverage we will provide you with a detailed billing summary which you can submit to your insurance company for reimbursement. Please note that there may be procedures and laboratory tests that are non-covered due to your individual policy/plan type. If you have any questions regarding your medical coverage, please call the telephone number on the back of your insurance card.
Regarding your Benefits
Medicare Information
Integrative Cardiology does not participate in the Medicare program. If you are a Medicare Part B beneficiary and wish to become a patient of Integrative Cardiology, you are required to accept the terms and conditions set forth in a Private Contract between you and Integrative Cardiology. This Private Contract provides that absolutely no Medicare payment will be made to you or to Integrative Cardiology for the services provided, even if such services are covered by Medicare. Under the Private Contract, you acknowledge that you accept full responsibility for the payment of charges for all services rendered by Integrative Cardiology.
